Posting the answer for anyone searching for this. After working with the Crutchfield tech it turned out to be a faulty on star adapting box, we got a new one and it works like a charm.

I'm installing my old D3 into my dads Chevy 1500 with On Star. I got all the wirring parts in from Crutchfield. I wired everything up and connected it before installing the radio in the dash. Everything worked when I connected the battery. I turned the truck off, installed the radio and put the dash back together. Nothing worked when I turned it on. I assumed one of the wires had come loose when cramming them in(not a lot of room back there). So today I pulled it out, soldered all the wires and plugged it back up. Presto, it came on.
