But where can you go to apply for the rebate? Is there a website or is it a mail in form?

The rebate was only valid for purchases between Oct 16th and Dec 31st.
I sent my rebate form on Oct 22nd, and my $200 rebate finally arrived today. Only took 3 and a half months, but I did get it!

Receive a $200 mail-in Rebate when purchasing a Pioneer AVIC-Z110BT,
or a $100 mail-in Rebate when purchasing a Pioneer AVIC-X910BT,
between October 16, 2009 and March 31, 2010, subject to the requirements below.

The dates may have changed, but the rebate form that I submitted was good for purchases between Oct 16th 2009 and Dec 31st 2009. Pioneer may have issued another rebate with extended dates...

Eight week turnaround. I mailed my rebate form on January 7 and got my check on March 5.
It's stupid that the check is basically a postcard, and not inside an envelope. Everyone can see that it's a $200 check with a signature block on the back. I won't be surprised if some get stolen out of the mail and forged.
